SUBJECT:
Requesting approval for the issuance of a Carnival License and a Music Festival to Our Lady of Mercy Church / Alpine Amusement Company, Inc. as part of our their Mercy Fest event held on October 16-19, 2014.

PURPOSE:
Our Lady of Mercy Church / Alpine Amusement Company, Inc. have applied for a Carnival license. Carnival to be held at Our Lady of Mercy Church on Thursday, October 16, from 5:00 p.m. to 10:00 p.m.; Friday, October 17, from 5:00 p.m. to 10:00 p.m.; Saturday, October 18, from 12:00 p.m. to 10:00 p.m. and Sunday, October 19, from 12:00 p.m. to 9:00 p.m. and Music Festival on Friday, October 17, from 5:00 p.m. to 10:00 p.m.; Saturday, October 18, from 12:00 p.m. to 10:00 p.m. and Sunday, October 19, from 12:00 p.m. to 7:00 p.m.

BACKGROUND:
This yearly event will include a Carnival, Music groups, food and games. Our Lady of Mercy Church has been holding this event for at least a decade and partnered with Alpine Amusement Company, Inc.

DISCUSSION:
The Applicant has provided the following required documentation for both the Carnival and Music Festival:
Carnival Application
Music Festival Application
Operational Permit Application from the Fire Prevention Bureau
Event Layout
Illinois Department of Labor Owner/Operator List for Rides
Emergency Procedures
Certificate of Insurance
The applicant will be submitting the required Health Department Permit, Performance bond, Food and Beverage Registration, Sponsorship letter, parking approval letter form Waubonsie Valley High School and security coverage prior to approval by the city Council.
