City of Aurora Ward Scholarship Template & Application.
BACKGROUND:
The Rules, Administration, and Procedures Committee requested a Ward Scholarship Template and Application be created to establish consistency and guidelines for all Wards who wish to offer a Scholarship Program.
DISCUSSION:
The recommendation of the Ward Scholarship Template is to require the following from each Applicant:
? Official transcripts from high school and/or accredited two-or four-year college.
*It is recommended but not mandatory that college students also include their high school transcript.
? Proof of past and/or present community service in Aurora, Illinois
? Proof of residence in Ward __, Aurora, Illinois
? Proof of enrollment in an accredited college or university for the academic year __.
? Proof of US citizenship or legal residency (birth certificate, etc.)
? Letter of Recommendation
? Personal Statement- "Why I am deserving of the Aurora - Ward ___ Scholarship"
? Photo of yourself and brief biography
? Applicant completed IRS W-9 forms, see www.irs.gov
? College/University IRS W-9 form
