DATE: July 17, 2018

SUBJECT:
A resolution authorizing payment to Advocate Occupational Health up to the amount of $200,000 annually for providing occupational health services.

PURPOSE:
To obtain approval for payment up to the amount of $200,000 annually to Advocate Occupational Health for occupational health services.

BACKGROUND:
Effective July 25, 2018, Northwestern Medicine will no longer be providing occupational health services and will be closing its facilities.
